A company has been fined after a night watchman fell into water at Inverkeithing, while a director has also been prosecuted for failing to carry out a risk assessment.

RM Supplies (Inverkeithing) pleaded guilty at an earlier hearing to breaching Regulation 4(1) and 5 of the Docks Regulations 1988 and was fined £14,000.

Furthermore, Thomas Muir, a director of for firm, pleaded guilty to breaching Regulation 3(1) of the Management Rules 1999 and was fined a total of £2,750.

It came after Charles Greenhill, 47, fell into water between the quayside and a berthing vessel. As there was a lack of adequate means of escape, the night watchman had to be rescued by the vessel's lifeboat.

Michael Orr, Health and Safety Executive inspector, said: "The conditions on site were unacceptable. There was no suitable or sufficient risk assessment, or attempt at planning which could have identified the state of the quay as unacceptable for use for any dock operations."

Earlier this month, a Shropshire boarding school was fined after a worker was killed while demolishing a building on the site, with investigators finding that no effective plan was in place.ADNFCR-2022-ID-800057978-ADNFCR
